I acknowledge that by filling out this consent form I have been given the full opportunity to ask any questions about obtaining a tattoo from Lacuna Studio, and that all my questions have been answered to my satisfaction. I fully understand the risks associated with getting a tattoo, and that these risks can lead to injury, included but not limited to infection, scarring, difficulties in detecting melanoma, and allergic reactions to tattoo pigment, latex gloves, and/or soap. Having been informed of the potential risks associated with getting a tattoo, I still wish to proceed with the tattoo application and I freely accept and expressly assume any and all risks that may arise from tattooing.

TO WAIVE AND RELEASE to the fullest extent permitted by law each of the Artist and the Tattoo Studio from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from the application of my tattoo. I agree to reimburse any attorneys’ fees and costs incurred in any legal action I bring against the Artist or the Tattoo Studio in which either the Artist or the Tattoo Studio is the prevailing party.

If I have diabetes, epilepsy, hepatitis, hemophilia, HIV-AIDS, or any other communicable disease, heart condition or take medicine which thins the blood I have advised the Artist and provided a doctor’s note saying I may proceed with the tattoo application. I am not pregnant or nursing. I am not under the influence of alcohol or drugs. I have informed the Artist of any allergies I am aware of.

The Artist and the Tattoo Studio will give me instructions for the care of my tattoo while it’s healing. I acknowledge that it is possible that the tattoo can become infected, particularly if I do not follow the instructions given to me. If I think I have an infection, I will consult with my doctor.

I acknowledge that the tubes are properly sterilized, that the needles are brand new with the expiry dates listed on all boxes, and that all the tattooing equipment at Lacuna Studio is properly maintained and used carefully. I am aware that the equipment used at Lacuna Studio can not cause any infections and that any probable causes are due to the client. 

I understand that variations in colour and design may exist between the tattoo art I have selected and the actual tattoo when it is applied to my body. I also understand that over time the colours and the clarity of my tattoo will fade due to unprotected sun exposure and the natural dispersion of pigment under the skin over time. I will check the spelling/placement of my tattoo once the stencil is placed and let the Artist know if any changes should be made prior to the tattoo being started. I understand that neither the Artist nor the Tattoo Studio is responsible for the meaning or spelling of the symbol or text that you wish to be tattooed.

I understand that a tattoo is a permanent change to my appearance and can only be removed by laser or surgical means.

I release all rights to any photographs taken of me and the tattoo and give consent in advance to their reproduction in print or electronic form. (Please notify your Artist if you prefer not to have a photo taken).

I acknowledge that I have been given adequate opportunity to read this document and understand that I am signing a legal contract waiving certain rights to recover against the Artist and the Tattoo Studio.

I acknowledge that I am over the age of 18 (we DO NOT tattoo minors).
